Hospice social workers play an important role in the lives of our patients. As a patient advocate, they care for and represent both the patient and their family throughout the end of life process. Our social workers aim to understand patient’s needs and desires, as well as those of their family.
Our social workers also communicate with medical staff to ensure that a patient’s physical comfort, emotional comfort, mental health, spiritual state, and the like are balanced and in good standing. Our social services including counseling, referrals, and coordination with other resources to help patients and their families prepare and make the necessary adjustments prior to death.
